Staying Well at Home
Our PCN’s Enhanced Practice Nurse and Care Coordinator work together to provide proactive, holistic care for housebound patients. They combine clinical support with personalised check‑ins to ensure people who can’t get to the surgery still receive regular reviews, wellbeing assessments, and help accessing other services. Read about their positive impact below.
Pet Therapy with our Social Prescribers
Rugby PCN’s Social Prescribers have recently introduced pet therapy sessions to support patients’ wellbeing. By bringing calm, friendly therapy animals into people’s homes, they help reduce loneliness, lift mood, and create a gentle space for conversation. This simple but powerful approach is already making a positive difference for housebound patients who benefit from comfort, companionship, and a reassuring connection to the wider community.
Occupational Therapy Service
Our Occupational Therapy team is helping to reduce hospital admissions by supporting patients to stay safe and independent at home. Through tailored assessments, practical advice, and early intervention, they identify risks before they escalate and put the right measures in place to prevent avoidable crises.
